Immunization History

Immunization History Screen

 Purpose

The Immunization History screen displays a clients immunization history and provides immunization recommendations based on the assigned tracking schedule. From this screen, you are also able to edit a clients immunizations, and/or pre-select immunizations to record for the client.

 

 Information Provided

The Immunization History screen displays a very large portion of the information held in the registry database for the selected client. It is possible, and can be useful, to print this screen.

The Immunization History screen displays a client information header titled Client Information at the top of the page. This header includes client demographics, selected tracking schedule, a scrolling list of client comments, as well as other client-distinct information.

The information contained in the header can also be used to confirm that you have located the correct client if you were brought to the Immunization History screen directly following a client search on the manage client/immunizations screen. The header also displays the clients VFC eligibility status.

If the client does not have a city recorded, a comment of "No Address Information on File!" will be displayed with a yellow background.

The client comments are visible at the top of the screen when you edit an immunization or when you add an immunization. When there is more than 1 comment just click on the up and down arrows to scroll through any available comments. When using the Print button on the immunization history page, the comments are displayed on separate lines in the Comments box.

The Immunization History table lists all vaccinations the selected client has received to date. Immunizations listed in the table are ordered alphabetically first, then by 'Date Administered'. The table columns are defined as follows:

Column Name

Description

Vaccine Group

This column lists the vaccine group name for each immunization received.

 Date Administered 

This column gives the actual day the client was given the vaccine. Click on the date hyperlink Oct-15-2001 to view the "Explanation of Dose Status" pop-up for that particular immunization. This pop-up window displays the tracking schedule rule applied to the selected dose for that vaccine series.

 Series 

This column denotes the sequence number within the immunization series.

 Trade Name

This column displays the trade name of the vaccination received. If an alternate vaccine group rule is in effect, the trade name will be prefixed by the name of the alternate vaccine group; for instance, "[Mumps] Mumpsvax"under the MMR vaccine group.

Dose

A column has been added to the Immunization History page and to client reports that displays the number of doses administered for each immunization.  This column reflects dose magnitude, not dose size as measured in milliliters.  For example if a pediatric dose were administered to a child the Dose column would display 'Full', if two pediatric doses were administered to an adult, the Dose column would display the number '2'.  In contrary, if an adult formulation were given to a child, the Dose column would display 'Half'.

WIR will also use this data to determine the validity of the vaccination provided.  For example a 'Full' pediatric dose being given to an adult would result in a not valid in the series column, because the dose magnitude needs to be '2'.

Owned Status

 

This column will display whether or not this shot was administered by your organization. If it is not owned there will be a hyperlink that says "No". You can click on the hyperlink to view which organization administered the shot as well as the contact information.

Note: If there is a question about an immunization administered by another organization we urge you to contact that provider with the supplied contact information.

 Reaction 

This column reports any reactions the client had after receiving a vaccine. A 'Yes' in this column indicates there was a reaction. To view the reaction, click the "Yes" hyperlink and a small information window will open, displaying the explanation of the selected reaction.

 Hist?

This column reports whether the recorded immunization was historical or not. A 'Yes' in this column indicates the immunization was recorded as being historical. If there is no 'Yes' then the immunization is new (out of inventory).

 Edit 

 

This column contains icons which, when clicked on, will allow you to edit the recorded immunization using the Edit Immunization screen.

*Note*

School entered immunizations will be shown with grey shading and bold yellow text.  These are deduplicated immunizations which have not been received by any non-school source.  School Entered immunizations with an associated reaction recorded will be shown with grey shading and bold red text.

The clients age displays in a solid blue field between the immunization history and before the recommendations, and also displays when you use the Print button on the immunization history page to print the history. The age is included as well on the Vaccine Administration Report in the box to the right of the clients name.

The recommended vaccinations and corresponding dates for the selected client reside in a table titled Vaccines Recommended By Selected Tracking Schedule at the top of the table. The vaccine group list appearing in this table represents all vaccinations included in the tracking schedule assigned to this client. The table columns are defined as follows:

Description

 Select

This column contains check boxes which can be used to pre-select any of the tracking schedule's immunizations when adding immunizations to the selected client. The check box will be disabled if the tracking schedule's recommended vaccination is contraindicated, or if all required immunizations for that vaccine group have been received.

 Vaccine Group

This column gives the recommended vaccine group name. Click on the vaccine hyperlink DTaP to view the "Explanation of Dose Status" popup for that particular vaccine recommendation. This popup window displays the tracking schedule rule applied to the selected recommendation for that series.

 Earliest Date

This column displays dates which note the earliest date the selected client could receive the corresponding immunization.

 Recommended Date

This column displays dates which note the date that the selected client is recommended to have the corresponding immunization.

 Overdue Date

This column displays dates which note the date that the client is past due for the corresponding immunization.

 Latest Date

This column displays dates which note the date after which the client should not receive the corresponding immunization.  If the recommended vaccination is contraindicated, then either “Contraindicated” or “immunity <to antigen> will display across the recommended date columns.

*Note*  If the recommended vaccination is contraindicated, then 'Contraindicated' or 'Immunity <to antigen>' will appear across the recommended date columns.

The dates found in the Vaccines Recommended by Selected Tracking Schedule table can be shaded with color for emphasis. The colors applied to the dates within the columns are defined as follows:

A red date and red border is a warning for the administration of combination vaccines. This may effect the schedule used for more than one series.

A red date and red border with a yellow background means that a client is within the Earliest Date to receive an immunization, yet warns that combination vaccines may effect the schedule used for more than one series.

Yellow will only be applied to those dates within the column Earliest Date. Dates with yellow shading mean that particular immunization has not yet been received, is equal to or past the earliest date, and is before the recommended date.

The color green will only be applied to those dates within the column Recommended Date. Dates with green shading mean that particular immunization has not yet been received, is equal to or past the recommended date, and is before the overdue date.

The color blue will only be applied to those dates within the column Overdue Date. Dates with blue shading mean that particular immunization has not yet been received and is equal to or past the overdue date.

 

 

The recommendations for vaccines now reflects whether a client has exceeded the maximum age or maximum doses for a specific vaccine. For instance, if a client has already reached the age of 5 and hasn't received any pneumococcal (Prevnar), then the recommendation for Pneumococcal at the bottom of the immunization history will show "Maximum Age Exceeded" and the client will not show on reminder reports as needing that immunization.

The 'Max Doses' label indicates the maximum number of doses that can be given for the specified series.

          

The 'Complete' label specifies the patient is complete for the corresponding vaccine. This means the patient will not display on reminder reports for this specific vaccine.

The 'Contraindicated' label indicates that the vaccine group should not be given due to a certain comment recorded on the client's record.

The 'History of Disease' label specifies the patient has a history of the disease for the specific vaccine group.

Complete: DPH Medical Director

The 'Complete: DPH Medical Director' label specifies an Authorized Override has been applied to the vaccine series and is considered complete.

 Functionality

For the most part, the Immunization History screen is for display purposes only. The functionality it does contain deals with linking to other pieces of the registry system.

Button

Description

When the Add Immunizations button is clicked, the Pre-select Immunizations screen is displayed, allowing you to verify and select which vaccines you are adding into the system for the client.

When the Edit Client button is clicked, the Edit Client screen is displayed containing the selected clients information so that updates can be made.

When the Reports button is clicked, the Reports Available for Client screen is displayed so that you may generate and/or print reports for the selected client.

When the Print button is clicked, the user will be taken to a screen where only the clients history and demographics are displayed. The menu on the left and top will not appear, and this is ideal for printing purposes.

Follow these steps to print:

Simply click the Print button, and the clients immunization history information will appear in a new window. Click on the printer icon in your browser or File>Print>All to print the page. Hit the Back button in your browser to return to the clients record.

When the Print Confidential button is clicked, the user will be taken to a screen where only the clients history is displayed. Client name, and date of birth will also display but no address will ever display. The menu on the left and top will not appear, and again this is ideal for printing purposes.

Follow these steps to print:

Simply click the Print Confidential button, and the clients immunization information will generate in a new window. Click on the printer icon in your browser or File>Print>All to print the page. Hit the Back button in your browser to return to the clients record.

When the Edit Immunizations icon is clicked inside the 'Immunization History' screen, the Edit Immunizations screen is displayed containing information on the selected vaccination so that updates can be made.

Selecting this button will launch the Blood Lead Program access code window where access can be gained to the Blood Lead Program.

Note:  When a user attempts to add an immunization to a client and the client does not have a City name saved for at least one responsible person's address, the user will receive a pop up message notifying them that "A City is required on at least one address. After entering and saving immunizations the user will be taken to the Edit Client page". After clearing the pop up, the user will proceed to enter immunizations as usual. After adding the immunizations, the user will be automatically directed to the Edit Client page and the Responsible Person tab. This notification will occur only once per client per user session.

 Pre-selecting Tracking Schedule Immunizations

The Vaccines Recommended By Selected Tracking Schedule table can be used to pre-select any immunizations included within the selected tracking schedule that the client has received when recording these immunizations into the registry.

  1. Select or un-select the desired immunizations to record by checking or un-checking the appropriate check boxes next to the desired vaccine groups within the table.

  2. Click on the Add Selected button.

  3. The Pre-select Immunizations screen will be displayed taking into account any vaccine groups checked from step one. Note: Only vaccine that you have available in inventory will become selected. If you are lacking some vaccine or you do not have inventory entered at all then you will notice not eveything will be checked on this screen.

  4. Continue as you would, following the Pre-select Immunizations screen steps.

 Viewing the Schedule Explanation Screen

The Schedule Explanation Screen is used to display the vaccination rule applied to a specific dose selected. This can aid in explaining why a dose was not valid or why a recommendation displays a particular date as examples. The window displays the table view of the schedule for the dose that was selected. For example, you are wondering why a dose of DTaP is marked as Not Valid. When you click on the vaccination date for that dose, a window pops up displaying the series for DTaP and includes a comment "Greater than the Maximum Age required for this dose" if this were the reason for being Not Valid. The selected dose in the series will be highlighted in yellow.

  1. For a specific dose in the history click on the date administered hyperlink in blue. (Oct-15-2001).

  2. A window will open displaying the series associated with the selected dose.

  3. If the dose is not part of a series in this schedule, a message indicating that the dose is not part of a series in this schedule will appear. Note: If this is the first dose given for a vaccine group and it is recorded prior to the minimum age, it will also give this message.

  4. When done viewing the information in the window, click on the 'X' in the upper right hand corner to close the window and return to the registry.

  1. In the Vaccines Recommended by Selected Tracking Schedule section on the history page, click on the vaccine group name, such as DTP/aP.

  2. The window will open displaying the schedule for the vaccine group.

  3. The selected dose in the series will be highlighted in yellow. Note: If a client had no immunizations in the series and was too old to complete, the first dose in the series would be highlighted.

  4. When done viewing the information in the window, click on the 'X' in the upper right hand corner to close the window and return to the registry.

 Notes

Any immunization record (row) that appears in the Immunization History table in all Red font indicates the client had a reaction to the corresponding immunization.
