Responsible Person(s) Tab

 Purpose

The Responsible Person(s) tab is used to display, add, delete, or update any person associated with a client as a responsible party.

This is the selected responsible person's relation to the client. Assign or change the relation by choosing a relationship from the 'Relation ' pick list.  There is a limit of 4 (four) responsible persons allowed per client and no duplicate relation types are allowed. * Note *

 Other Fields

Field Name

Description

Last Name

This is the selected responsible person's last name. Assign or change a last name by typing the desired name into the 'Last Name' text box.

First Name

This is the selected responsible person's first name. Assign or change a first name by typing the desired name into the 'First Name' text box.

Middle Name

This is the selected responsible person's middle name. Assign or change a middle name by typing the desired name or initial into the 'Middle Name' text box.

Telephone

This is the selected responsible person's telephone number. Assign or change the telephone number by typing the numbers into the 'Telephone' text boxes.

Extension

This is the selected responsible person's telephone number extension. Assign or change the extension by typing the number into the 'Extension' text box.

E-Mail

This is the selected responsible person's email address.  Assign or change the email address by typing the email address into the 'E-Mail' text box.

Street Address

This is the selected responsible person's street address information. Assign or change the street address by typing the numbers and/or letters into the 'Street Address' text box. For example: 1520 Main Street.

Note: If you choose to enter apartment information on this line, it is extremely important to precede it with the Apt abbreviation. For example: 1520 Main St Apt 455

Other Address

This is the selected responsible person's other address information. Assign or change any additional address information by typing the numbers or letters into the 'Other Address' text box. For example: Apt. # 6.

P.O. Box

This is the selected responsible person's P.O. Box number. Assign or change the P.O. box number by typing the numbers into the 'P.O. Box' text box. For example: 3036.

City

This is the city in which the selected responsible person currently lives. Assign or change the city by typing the name of the city into the text box directly following 'City'.

State

This is the state in which the selected responsible person currently lives. Assign or change the state by selecting the desired state from the 'State' pick list.

Zip

This is the ZIP code in which the selected responsible person currently lives. Assign or change the ZIP code by typing the five-digit number into the 'ZIP' text box.

+4

This is the extension of the ZIP code in which the selected responsible person currently lives. Assign or change the ZIP code extension by typing the four-digit number into the '+4' text box.

Language

This is the language the selected responsible person speaks. Reports and notices will then be generated in the chosen language.  The default language for responsible person is English.

Notices?

This check box is used to determine whether the selected responsible person will be sent reminder/recall notices. If the box is checked, the responsible person is eligible to receive the notices. (This depends upon whether or not the 'Allowed Contact' field on the Client Information Tab is checked, and whether or not they have sufficient address information.  The client's status must also not be set to 'Permanently Inactive - Deceased' on the Client Information tab.) If the box is unchecked, they will not receive notices.

Primary?

This check box is used to determine whether address of the responsible person is primary address of the client.  Only one responsible person can be set as primary.  If the check box is NOT selected, the most recently updated address is defaulted to the primary address. The Primary address is the address that is displayed on the header on the client screens.

 Address Validation

When you save the set of addresses for the client's responsible persons, the system will check to see whether they are valid addresses. The system uses Street Address, City, State, and 5-digit zip code to determine validity through comparison with the reference data from the U.S. Postal Service. Here are the possible outcomes.

Address Status

Description

Address not found

The System has not found any matches for the address entered. 

Address validated to zip code

The System cannot find any street-level matches but has determined the city/zip code is valid.

Multiple matches found; address validated to zip code

The System has found more than one possible street-level match for the address entered.

Address validated to city
 

The System has not found any street-level matches, but it has determined that the city/state combination is valid.

Primary number is missing
 

The System has found a possible street-level match but the primary street address number is missing.

Primary number is invalid
 

The System has found a possible street-level match but the primary street address number is invalid.

PO, RR, or HC box number is missing

The System has found a possible street-level match but the primary PO Box, Rural Route, or Highway Contract Box number is missing.

PO, RR, or HC box number is invalid

The System has found a possible street-level match but the primary PO Box, Rural Route, or Highway Contract Box number is invalid.

Secondary number is missing 

The System has found a street-level match but address is missing secondary number (APT, STE, Etc.).

Private mailbox number is missing 

The System has found a street-level match but address is missing private mailbox number (PMB).

Vacant address

The System has found a street-level match for the address entered, but it was identified as vacant. Mail has not been collected for 90 days or more at this address.

Inactive address

The System has found a street-level match but this address is not considered deliverable by US Postal Service.

Confirmed address by dropping secondary number  

The System has found a street-level match by dropping the secondary number (APT, STE, Etc.) from the address.

General Delivery address  

The System has found a street-level match but it is for a General Delivery address (I.E. Local Post Office).

Unique zip code 

The System has found a street-level match but found inconsistencies in the last four digits of the zip code.

Address is deliverable 

The System has found a single correct match for the address entered.

WIR only accepts alpha characters, dashes, and apostrophes.

First Name

WIR only accepts alpha characters, dashes, and apostrophes.

Middle Name

WIR only accepts alpha characters, dashes, apostrophes, and periods.

Telephone

WIR only allows numeric characters.

E-Mail

WIR will only accept E-Mail addresses that contain '@' and a period.

Street Address

WIR allows alpha or numeric characters, dashes, periods, apostrophes, forward slashes '/', and pound symbols '#'.

Other Address

WIR allows alpha or numeric characters, dashes, periods, apostrophes, forward slashes '/', and pound symbols '#'.

P.O. Box

WIR allows alpha or numeric characters, dashes, periods, forward slashes '/', and pound symbols '#'.

City

WIR only accepts alpha characters, dashes, and apostrophes.

Zip

WIR only allows numeric characters.

 Notes

When a user saves a new or existing client record and the client does not have a City name saved for at least one responsible person's address, the user will receive a pop up message notifying them that "A City is required on at least one address". After clearing the pop up the user will be automatically directed to the Responsible Person tab. This notification will occur only once per client per user session.

In order for the registry to create and maintain a record for a client, school, etc., there is a minimum amount of information necessary that must be provided by a user. These select pieces of information are the Required Fields and are denoted in the registry by the blue information field labels as seen in the example below.
